What is neck pain?
Your neck, called the cervical spine, is made of seven small vertebrae stacked between the skull and the upper back. Discs cushion the space between them, small joints guide every turn and tilt, and nerves leave the spine at each level to serve the shoulders, arms, and hands.
Most neck pain is mechanical. It comes from how those joints, discs, muscles, and nerves are moving and bearing load, not from a disease. It is also very common. The authors of a major neck pain trial in Annals of Internal Medicine described it as "a common condition that affects an estimated 70% of persons at some point in their lives" (Bronfort et al., 2012).
It can show up as a stiff neck, a sharp catch when you turn your head, neck and shoulder pain with tight muscles across the top of the shoulders, headaches that start at the base of the skull, or pain and tingling that travel into the arm or hand. Mechanical problems call for hands-on, movement-based care, and that is what chiropractors are trained to provide.
What causes neck pain and a stiff neck?

What the research says about chiropractic care for neck pain
In a randomized trial, spinal manipulation was more effective than medication. Researchers randomly assigned 272 adults, aged 18 to 65, with neck pain lasting 2 to 12 weeks to one of three groups: spinal manipulation therapy (SMT) from a chiropractor, medication, or home exercise with advice. The results, published in Annals of Internal Medicine: "SMT had a statistically significant advantage over medication after 8, 12, 26, and 52 weeks." At 12 weeks, 56.7% of the spinal manipulation group reported at least a 75% reduction in pain, compared with 33.3% of the medication group. The authors concluded that SMT "was more effective than medication in both the short and long term." Home exercise with advice also performed well in that trial (Bronfort et al., 2012).
National best-practice recommendations call for it. In 2019, a national panel of 56 chiropractors with expertise in practice, research, and teaching published best-practice recommendations in the Journal of Manipulative and Physiological Therapeutics (JMPT). Their conclusion: "For uncomplicated neck pain, including neck pain with headache or radicular symptoms, chiropractic manipulation and multimodal care are recommended" (Whalen et al., 2019). Multimodal care means the adjustment combined with exercise and other supportive therapies.
Older adults did better, at lower cost. In a randomized trial of 241 adults aged 65 and older with chronic neck pain, spinal manipulation plus home exercise produced greater pain reduction after 12 weeks than supervised exercise plus home exercise or home exercise alone, and no serious adverse events resulted from the study treatments (Maiers et al., The Spine Journal, 2014). In a cost analysis of the same trial, total costs for the spinal manipulation group were 47% lower than for the supervised exercise group, and the authors wrote that spinal manipulation plus home exercise "resulted in better clinical outcomes and lower total societal costs" than either alternative (Leininger et al., The Spine Journal, 2016).
Patients with a herniated disc in the neck reported improvement. A prospective Swiss study followed 50 patients with neck and arm pain from a cervical disc herniation confirmed on MRI, all treated with spinal manipulation by doctors of chiropractic. At two weeks, 55.3% reported being improved. At one month it was 68.9%, and at three months it was 85.7%. The authors noted that most patients "reported significant improvement with no adverse events." This was an observational study without a comparison group, so it describes what patients experienced under care rather than proving cause and effect (Peterson et al., JMPT, 2013).

Medication, injections, and surgery have their place, and when an exam shows that a patient needs them, we say so. For most neck pain, the research supports starting with conservative chiropractic care.

Is a chiropractic neck adjustment safe?
The research is reassuring. The trial in older adults reported no serious adverse events from the study treatments, the cervical disc herniation study reported no adverse events, and in the Medicare study of 291,604 patients, chiropractic care "was associated with lower rates of adverse events than primary medical care." A population-based study of vertebrobasilar artery (VBA) stroke in Ontario found that "VBA stroke is a very rare event in the population" and concluded, "We found no evidence of excess risk of VBA stroke associated chiropractic care compared to primary care" (Cassidy et al., Spine, 2008). Some patients feel mild soreness after an adjustment, much like after exercise, and it passes quickly. Your exam screens for the rare cases where a different approach is needed first.
